In the summer of that year, Chekhov happened to work at the Chikinskaya zemstvo hospital. The story is built around dialogues. The speech of one of the characters, the church clerk Vonmiglasov, who comes to have a painful tooth removed, is filled with Church Slavonic phraseological expressions. The feldsher Kuryatin, who is substituting for the doctor, tries to speak briefly, concisely, and laconically.
